Our Mission
Founded in 1993, the Primary Care Development Corporation (PCDC) is a nonprofit organization dedicated to ensuring that every community has timely and effective access to primary care. Our goal is to increase the capacity and quality of primary care in underserved communities to achieve our vision of "Excellent Healthcare in Every Neighborhood."
Our Strategy
To achieve its mission, PCDC relies on three strategies aimed at improving health outcomes, reducing health disparities, and lowering healthcare costs in communities where primary care is needed most. These strategies are:
Investing in primary care facilities. We provide the capital to build and modernize facilities, expand practices, and upgrade outmoded equipment, leading to better services for more patients.
Strengthening service delivery. We guide providers to develop patient-centered approaches that strengthen their practices and improve and expand care.
Leading policy initiatives. We promote policies that increase access to quality primary care, improving the health of communities and lowering health system costs.
The PCDC Story
1993: INVESTING...
PCDC was founded to fix a major health systems problem - a critical lack of access to primary care physicians and services in New York's underserved communities. Under Mayor David Dinkins, New York City made available tax exempt bond financing while a group of private foundations provided startup funding for PCDC's Capital Financing services. This program provides critical financing to construct new primary care facilities and upgrade and expand existing ones. PCDC, in partnership with five major banks and public sources, continues to offer a variety of financing tools to health centers throughout New York State. To date, PCDC programs have created an investment of $255 million in over 84 health center projects while leveraging $6 of private investment for each $1 of public investment.
1998: STRENGTHENING...
As we worked closely with providers, we recognized the importance of helping practices maximize their existing capacity and staff. In 1998, we began to deliver Performance Improvement services designed to help providers redesign practices and implement new management systems to serve more patients and deliver higher quality care. To date, PCDC has delivered these programs to over 400 primary care practices throughout New York and increasingly throughout the United States.
2005: LEADING...
As leaders in New York State and throughout the country increasingly recognized the need for investment in primary care as a means to control healthcare costs and reduce healthcare disparities, PCDC began Policy and Advocacy initiatives to strengthen primary care. PCDC is a co-founder of the New York State Primary Care Coalition and works in partnership with government leaders to expand access to quality primary care in underserved communities.
2009: BROADENING IMPACT...
With a focus on sustainability and measurable impacts, PCDC offers a model for primary care development that can benefit communities throughout the United States. Successful primary care development models are needed as healthcare reform increases demand for primary care.
