All of PCDC's programs and services focus on: achieving our organizational mission; delivering high-impact and measurable results; providing highly professional expertise; and ensuring that our client's results are sustainable. We are committed to:



Investing in primary care facilities. PCDC is the only Community Development Financial Institution (CDFI) in New York State specializing in primary care, and the first and largest in the U.S. PCDC's Capital Financing program was developed to address health centers' needs for financing, as it posed a major barrier to primary care expansion. Enlisting the support of city, state and federal government and major banks, our financing programs have been responsible for building primary care capacity in underserved communities across New York State. These financing program have:

  • Created total investments of $260 million for 91 primary care projects. Nearly all of this investment is in areas of highest priority for both healthcare and economic development need.

  • Expanded primary care capacity to provide 1.75 million medical visits annually, meeting the primary care needs of more than 595,000 underserved New Yorkers.

  • Transformed 655,000 square feet of space into fully functioning primary care practices

  • Created 1,300 construction jobs and 2,300 permanent jobs in low-income communities




Strengthening Service Delivery. PCDC's Performance Improvement team has worked with more than 400 health center teams to implement effective, patient-centered and economically sustainable business practices to expand access and delivery of high-quality care in underserved communities.

  • We have helped health centers cut appointment (cycle) times by 50%; reduce wait times for appointments from 21 to 0-5 days; and decreased patient no-show rates by 67% on average.

  • Increased emergency readiness at 77 primary care sites by 90% (in accordance with New York City Department of Health standards)

  • Leading eight Brooklyn test sites to develop a replicable and adaptable Electronic Medical Record (EMR) implementation model through the Primary Care Health Information Consortium (PCHIC)

  • Selected to provide technical assistance to the Commonwealth Fund Safety Net Medical Home Initiative, and participating in the Emblem Health Medical Home High Value Network Project to help primary care practices gain Patient-Centered Medical Home recognition.




Leading Policy Initiatives: PCDC plays a key leadership role in Policy & Advocacy efforts to strengthen and expand primary care in underserved communities, including Patient-Centered Medical Home (PCMH) implementation, Medicaid reimbursement reform, capital investment, and quality standards for primary care.

  • Awarded with the Wachovia NEXT Award, which recognizes CDFIs that have exceptional track records and extraordinary potential. PCDC received the award in recognition of its advocacy efforts on behalf of primary care in New York State.

  • Co-founded the Primary Care Coalition, which brings together five major statewide primary care organizations (Community Health Care Association of New York State, New York State Area Health Education Center, New York Chapter of the American College of Physicians, and New York State Academy of Family Physicians) to achieve affordable, appropriate healthcare coverage for all through the enhancement, support, and sustainability of primary and preventative care services in New York State.

  • Worked to ensure that the Commission on Health Facilities in the 21st Century (Berger Commission) included primary care recommendations.

  • Helped secure $600 million in primary care investments and reforms for New York State

  • Helped secure $200 million in capital funding to expand primary care capacity and assist primary care providers in adopting information technology.

  • Founding member of NYC City Council Speaker's Primary Care Task Force, and helped develop the Speaker's Primary Care Initiative.

  • Semifinalist for New York Times Company Nonprofit Excellence Award (2009)
