About Performance Improvement


PCDC helps practices deliver high-performing, patient-centered primary care. Using proven strategies, we work side-by-side with our clients, helping them assess and understand their operational issues; develop customized strategies for change; define measurable outcomes; and establish processes to sustain these improvements.



Meeting Your Needs



We have worked with more than 400 primary care teams in underserved communities throughout the country, guiding them through the redesign of their operations so they can dramatically expand access to quality primary care. Our services include:


Practice Redesign: improve access to care through improvement in patient visit wait times (cycle time), appointment scheduling, provider productivity, and patient and staff satisfaction.


Emergency Preparedness: plan for emergencies ranging from blackouts to pandemic flu by creating emergency response plans, conducting emergency drills and participating in regional planning exercises.


Meaningful Use of Health Information Technology: integrate technology to enhance quality of care, coordinate care across provider settings, use patient data, and improve patient-provider communication.


Medical Home Recognition:adopt new processes to achieve Patient-Centered Medical Home (PCMH) recognition.

Implement proven strategies. We employ strategies that improve quality and support timely and effective care for every patient, and build organizational capability for continuous improvement and sustainability.


Coach and educate. We work closely with multi-disciplinary teams, from nurses to finance departments, from Medical Directors to CEOs. We engage in intensive coaching and a variety of training and facilitation methods that promote education, communication, and sustainability.


Produce measurable results. We encourage continuous improvement and promote sustainable outcomes in patient access and care.
